VOLUNTEER CHAPERONE APPLICATION PROCEDURE
Please ensure that you have followed the correct procedure (below) for submitting your application online and have watched the absolutely captivating training video on “How to Be a Chaperone”.
· If you have never chaperoned at Mohave specifically, you’ll need to do the application AND the video.
· If you’ve chaperoned at Mohave within the last three years, you’ll just need to watch the video.
· If it’s been more than three years, you’ll need to do the application AND the video.
· If you are a district employee, you do not have to do the application, but you do have to watch the video. (Yeah, every year).
1. Visit www.SUSD.org
2. Click on the "About" tab at the top of the page
3. Click on "Volunteers"
4. Click on "The Volunteer/Chaperone application is now online" link
5. Click on the "Apply for this position" tab in the top right corner
6. Fill out the application
7. Save and Attach it to an email and send it to Julie Brown at [email protected], with the Subject “Parent Volunteer Chaperone for Mohave Middle School”. (Please CC me on the email).
8. Include the name of your student and their class (band/strings/choir) in the email body.
9. Go back to the "Volunteer & Chaperones" webpage and watch the video (input name, phone #, email, school - and click SUBMIT) to register to be a chaperone.
The system will not notify us if you fill out the form, so do not forget to attach application to an email to Julie Brown ([email protected]) and email me that you’ve finished the process ([email protected])

***TURNING IN MONEY for California:
- Turn in payments to Julie Brown in the front office.
- Make sure student's name and what group they are travelling with (Choir) are put on all payments.
- Make checks payable to Mohave Choir (so she knows what group to put the money into)
- Please put student ID# on all checks!

TAX CREDIT PAYMENTS FOR TRIP:
Follow this link to make a Tax Credit payment for the CA Trip: touchbase.susd.org/taxcredits/
NOTE: Be sure to select "Mohave Middle School" and then under "Item"- select "Choir Disneyland Trip" and be sure to put your student's name in the next box.
